(Please either: type to fill out and print, or print and fill out legibly with a pen. Upon printing, please sign and date, and either mail or submit in person your form to the Amherst County Sheriff's Office.)
  1. Citizen Feedback Form - Used to either formally commend a deputy or to file a formal complaint.
  2. Concealed Carry Permit Application - (Link to VA State Police Form) (Submit to Clerk of Circuit Court)
  3. Eviction Procedures - Step by step guide for the eviction process
  4. Freedom of Information Act Request - Use to obtain copies of department information that is subject to the Freedom of Information Act. This form should be forwarded to Major Betty Wise (FOIA Officer) at btwise@amherstsheriff.org or 115 Taylor St, Amherst, VA 24521.
  5. Precious Metals & Gem Dealer Application - Required by County Code prior to opening a business of this type.
  6. Ride Along Application - Required to participate in the Sheriff's Office Ride Along Program. Must be accompanied by the appropriate waiver. Ride Along Waiver - Adult or Ride Along Waiver - Minor. Remember to fill out and turn in both the Ride Along Application and the appropriate waiver.

Fingerprinting

The Amherst County Sheriff's Office will provide fingerprinting to the general public on Mondays (except observed holidays) from 1:00 pm to 2:00 pm at the Amherst County Courthouse. It is suggested that you call ahead to ensure the staffing is available to provide this service. The service is provided for free to Amherst County residents (ID is required to prove residency) and $5.00 for non-residents. The $5.00 fee (if applicable) must be paid at the Sheriff's Office prior to fingerprinting. If you have any questions please contact us 434-946-9379.
